Duty of Care form

A duty of care form is a legal document that needs to be completed every year. You'll need your customer number which was provided to you in your duty of care email. 

5 easy steps to complete your annual Duty of Care form

1. Click on the service you have using the following link - refuse collection (if you have more than one then you will need to complete a form for each service)
2. Input your customer number (which is in your email reminder)
3. Check you have the correct business name and address.
4. Use the waste type list to select the types of waste that are disposed of in your bin, you can add more than one waste type for your bin. Please ensure you add a waste type for every type of waste that will be disposed of in the bin.
5. Input your email address in the space provided and press: proceed to payment (if your details have changed, then please call the team on 01773 841453 who will advise you what to do next). 

Please note that there is an administration fee of £20 for each duty of care waste transfer note to be emailed to you.

After you have paid the £20 fee the documentation will be emailed to you within 10 working days.

You have then completed your annual duty of care form for 2026/27.

Polite reminder - can customers ensure their 2026/27 duty of care form is completed before 1 April 2026.
